For the purposes of the Data Protection Act 1998, Lee Wickham operating under the name of Fascia First Chiropractic (‘we’ or ‘us’) is responsible for the controlling and the processing of your personal data ( ‘Data Controller’) for Chiropractic Care purposes. Other parties may process data you have provided, such as Kiku Notes, Square Appointments, Square Payments and WordPress. All carry their own privacy and cookie policies distinct to this one.

Personal Data is data that relates to a living identifiable individual. This data includes things such as your name, address, telephone number, email address and payment details including sort code and account number. We collect the following Personal data you:

3. Personal data we may collect about you
We may monitor your usage of this website. This may include monitoring how many times you visit the website, which pages you go to, traffic data, location data and the originating domain name of a user’s internet service provider. This data will be aggregated or statistical, which means that we will not be able to identify you individually.

- To assess and review your progress as an active patient

5. Disclosure of your personal data
In booking to attend a session your data will be processed using Square Appointments Platform. this data may be entered online by you or by me. This is the same platform as the payment gateway. Once Treatment is embarked the progress Notes platform Kiku will be used to keep your progress notes safe.
We may also disclose personal data to Law enforcement agencies, in connection to any investigation, in order to help prevent unlawful activity. If our association is integrated with another entity your details may be disclosed to our advisers and their advisers and will be passed on to the relevant department/s within the new entity subject to terms of confidentiality no less onerous than those contained in this policy.
6. Keeping your Personal data
We will hold your personal data on our systems for no longer than as is necessary (usually 8 years) without limitation in line with the requirements under the Data Protection Act 2018, and for the sole purpose of our associated contact with you. We will hold only information relevant to our association to enable us to conduct treatment, referrals and/or to provide Expert Witness advice to my regulatory body.
